Diobox offers Survey Forms designed for collecting guest information with event-specific attributes. Each guest receives a unique survey link, ensuring they can only submit their own response.

The Guest Experience During the Survey Process
Guests receive an email with a unique survey link. Clicking the link takes them to the survey page, where they can answer one or more questions. The unique link ensures that only the invited guest can submit the survey, preventing it from being shared with others.

Create a New Survey Form
To create a new survey form, navigate to the Forms
tab and click the '+'
icon at the upper-right corner of the page.
Choose Survey from the list.
Set Form Availability & Attributes
You can control the following attributes on your Survey Form:
Name and Description: The form name and description will appear on the confirmation page.
Closing Time: Specify when the form closes. By default, the form will close at the start of your event. Leave this field blank if you wish the form to remain open indefinitely.
Customizing Your Form
With the Diobox Form Builder, customize your form to enhance both aesthetics and functionality. Add custom questions and content to enrich the user experience during the registration process.

Add Survey Questions
While editing the survey in the Form Builder, drag a Question block to the desired position.
Then, you will be presented with all available event questions. You can either add an existing question or create a new one.
Capture Additional Registration Fields
By default, the survey form includes the guest’s name and email address at the top, indicating who the survey belongs to. These fields are not editable by the guest.
If you want guests to update their primary registration fields (e.g., company name or job title), click the header and select additional fields to include in the survey.
When guests access the survey, fields with existing data will be pre-filled, allowing them to update their information if necessary.
Submission Message
After guests successfully submit the survey, you can display a custom message. This can include follow-up steps, further instructions, or a thank-you message.
You can customize the Submission Message in the Form Attributes section.
Submission Email
When a survey is submitted, the guest and your team members will receive a notification email with the survey’s content and a receipt link.
You can customize or disable this email by navigating to Forms > Email Responders
.
Tracking Responses
You can track survey responses in multiple ways:
1. Guest Page
Individual responses are visible under the Activity section of the guest’s profile.
2. Submission Page
Navigate to Submissions > Questions & Responses
to view a summary of responses, including charts and detailed breakdowns of individual guest answers.
